# I would like to submit a change

## As a customer of Therapieland or Gezondeboel, you can submit changes using one of the forms below.

For example, you can let us know when a new professional starts or that there are changes in the practice or organization.

We ask you to only use these forms if you are authorized by your organization to report changes. As an applicant, we will also keep you informed of your application.
