      I would like to submit a change

      As a customer of Therapieland or Gezondeboel, you can submit changes using one of the forms below.

      For example, you can let us know when a new professional starts or that there are changes in the practice or organization.

      We ask you to only use these forms if you are authorized by your organization to report changes. As an applicant, we will also keep you informed of your application.

      Organizational changes

      Report changes here with regard to your organization or practice (address, e-mail, chamber of commerce number, contact person).

      Change professional accounts

      Submit changes here to activate or deactivate professional accounts. We then implement the changes.

      Request new services

      Submit requests for additional services or programs using this form. We will then add it for you.
