Care Plan - user guides for clients

On the Care Plan page you will find all treatments and questionnaires you have been invited to work on. From here you can start working on a treatment or continue from where you left off.

Different care plans for different organisations

When you accept an invitation from a professional, a care plan with the name of the organisation this professional is connected to will appear in your account. There is a care plan for every organisation you are connected to. The number of care plans you have is indicated at the top of the page.

When you have created selfhelp account you will also see a careplan named Therapieland or Gezondeboel. You can add treatments to this care plan yourself. In most cases you will pay a set amount for those treatment. When you were invited by a professional treatments are free of charge.

Who has access?

On the right side of the page, next to the name of the care plan, you can find a list of professionals that have access to that specific care plan. If you have more than one care care plan, these lists will be different for every care plan.

Treatment Goal

Your professional can set a goal for the care plan, to motivate you for example. This Goal is visible on the care plan page in the column on the right. When your professional hasn't done this, under Goal it will simply say: There is no goal set.
